Your Checklist for Your First Vein Appointment

Find out how to prepare for your first appointment with a vein specialist, and what to expect once you arrive.

Nearly 40% of Americans are affected by vein disease, otherwise known as chronic venous insufficiency, but only a fraction seek treatment. If you’re one of the millions who have varicose veins, spider veins, or other conditions resulting from venous insufficiency, setting up an appointment with a vascular medicine specialist is the first step to seeking relief.

Your initial appointment is a chance to meet your doctor and learn about the different treatment options available to you. You can expect your meeting to take approximately two hours, including a primary examination. During this time, your doctor and team of specialists will evaluate your needs and work with you to develop a personalized treatment plan.

To prepare for your appointment, drink plenty of fluids, dress warmly, and bring loose-fitting shorts to wear during your leg examination. You’ll also need to bring a few documents for our records, including:

Photo ID (Driver’s License, State ID, Military ID, etc.)

Current insurance card(s)

Referral slip from your Primary Care Physician (if required by your insurance)

Before arriving at your appointment, you can expedite the registration process by downloading and completing the following new patient registration forms. These forms include:

Patient Information Form: this form gathers your personal and insurance information for our records.

Medical Information Form: this captures your medical history and health information, including all medications and supplements you’re taking and any allergies you may have.

Patient Privacy and HIPAA Protection Form: this explains our compliance with the Health Insurance Portability and Accountability Act (HIPAA) Privacy regulations, and that our Notice of Privacy Practices is available for review (on our website and at registration). We also require your acknowledgement of certain authorizations and consents.

Communication Preference and Messages Agreement Form:this form allows you to specify the best way for your doctor to communicate with you in the future.

Patient Consent, Assignment of Benefits and Acknowledgement Form: this includes your consent to treatment and assignment of insurance benefits and payment. It covers our general patient financial agreement and no show/cancellation policy as well.

Practice Business Policy: this form goes over our business and financial policy, and your responsibility relative to payment. It also addresses insurance or physician referrals.

Setting up your initial consultation appointment is an important part of finding relief from pain and unsightly veins. By working with your doctor to address both the symptoms and root cause of your leg discomfort, you’ll be on your way to looking and feeling better.
