Many people notice varicose veins before they feel them. Those rope-like, bluish or purple veins that bulge and twist along the legs are hard to miss. But the veins you can see are often only part of the story. Many people also live with aching, heaviness, swelling, or nighttime leg cramps and never connect those feelings to the veins on the surface.
Here is the part that matters: varicose veins are usually a sign of a deeper problem with how blood moves through your legs. When you understand what is causing the discomfort, you can do something about it.

Varicose veins are enlarged, twisted veins that usually appear on the legs and feet. They form when the veins stretch and swell due to reflux (backwards flow-more on this later), causing them to press against the skin. Doctors define them as bulging veins that measure three millimeters or wider (about the thickness of a coffee stirrer). They are very common. According to the National Library of Medicine (NIH), varicose veins affect up to 40% of adults, and the risk increases with age.
Veins have an important job. They carry blood back up to your heart, working against gravity the whole way. When you walk, your muscles squeeze the veins, which propels the blood. You can think of your calf muscles as a peripheral heart. To keep blood moving in the right direction, veins have tiny one-way valves inside them. When those valves work well, blood flows up, and the valves snap shut so the blood cannot fall back down. Varicose veins are a sign that this system is not working as it should.
Varicose vein symptoms tend to develop gradually, and they often worsen at the end of the day or after long periods of standing or sitting. According to the American Heart Association, early symptoms usually appear around visible veins and can include aching, throbbing, burning, itching, and dry, irritated skin. Most varicose veins trace back to a condition called chronic venous insufficiency (CVI). That sounds fancy and is a mouthful, so here is the plain version: it means the one-way valves inside your leg veins have become weak or damaged, so they no longer close properly.
When those valves fail, blood does not flow smoothly back up to the heart. Instead, some of it leaks backward and pools inside the vein. Vein doctors call this backward flow reflux. Over time, the trapped blood raises the pressure inside the vein, stretching the vein wall and creating the bulging, twisted shape you can see on the surface.
This is the key point that many people miss. The visible vein is not really the whole problem. It is the result of a valve issue happening deeper in the leg. That is also why the same faulty valves that cause the bulge can cause the aching, swelling, and heaviness at the same time. Treating just the surface vein without addressing the underlying valve problem often results in the symptoms returning and progressing.
Anyone can develop varicose veins, but some people are more prone to them than others. Research points to a few common risk factors, including age, family history, and pregnancy.

It is a common belief that varicose veins are just a cosmetic issue. For some people, that is true. But for many, the visible veins point to venous insufficiency that can slowly get worse if it is ignored.
When symptoms are left unchecked for a long time, skin changes such as discoloration and thickening can become permanent, and open sores can develop near the ankles. That is why timing matters. Treating symptoms earlier, when you first notice aching or swelling, usually leads to a better outcome than waiting until the skin has already changed.
It is a good idea to see a CVR vein specialist if you notice any of the following: leg pain or heaviness that interferes with daily life, swelling that does not go away, skin around the ankle that is changing color or texture, a vein that becomes warm, red, or tender, or any sore that will not heal. You cannot diagnose varicose vein disease by looking at the surface alone. The size of a bulging vein does not always match the severity of the underlying valve problem, and some people have significant vein disease with only mildly bulging veins.
The standard test is a duplex ultrasound, a painless scan that uses sound waves to create a picture of your veins and show how blood is moving through them. It lets the specialist see which valves are leaking and map the exact path of the problem. Duplex ultrasound is considered the gold standard for diagnosing vein disease in the legs, because it shows the specialist what is happening beneath the skin, not just what is visible on top. This is important because a clear diagnosis is what makes the right treatment plan possible.
Treatment for varicose veins has come a long way. Office-based minimally invasive procedures have largely replaced the older approach of surgically stripping out veins. These treatments use tiny openings instead of large cuts. Done without general anesthesia, these modern procedures allow you to drive to and from appointments and return to your normal routine quickly, usually the same day
The goal of these treatments is to close off the faulty vein so blood naturally reroutes to healthy veins nearby. Once the damaged vein is closed, pressure in the leg drops and symptoms tend to ease. Common options include:

A five-year study published in the New England Journal of Medicine found that minimally invasive options such as laser ablation are recommended as alternatives to traditional surgery, with strong long-term outcomes. Which treatment is right for you depends on the size and location of your veins and what the ultrasound shows, which is why an evaluation comes first.
Along with these procedures, many people find relief from compression stockings, which gently squeeze the legs to help blood move upward. Compression can ease symptoms and is often used as a first step or alongside other treatments.

However, they can be a sign of chronic venous insufficiency, which may worsen over time and lead to skin damage or slow-healing sores if left untreated.
