Center for Vein Restoration (CVR) is proud to welcome Caroly Guillen Aguirre, MD, a fellowship-trained vein specialist and board-certified internal medicine physician, to our vein clinic in Waldorf, Maryland. Dr. Guillen Aguirre brings advanced, minimally invasive vein care to patients throughout Charles County and the wider Southern Maryland region.

Dr. Guillen Aguirre's foundation in internal medicine shapes how she cares for every patient. That training taught her to look past any single symptom and consider a person's whole health before recommending treatment. Paired with focused fellowship training in venous and lymphatic medicine in CVR's prestigious fellowship program, it gives her a style of care that is thorough, evidence-based, and genuinely personal.

For people across Southern Maryland who have spent years living with leg heaviness, swelling, varicose veins, or aching they assumed was just a normal part of getting older, Dr. Guillen Aguirre is ready to help. And because she is fluent in Spanish, Spanish-speaking patients can talk through their symptoms, questions, and options directly with their physician, in their own language.

I want to treat every patient the same way I would want my own family, or myself, to be treated. Listening, compassion, and empathy matter because we are caring for people's lives.

A Path from Internal Medicine to Vein Care

Dr. Guillen Aguirre's path to vein medicine started well before she arrived in Maryland. She grew up in Venezuela and earned her medical degree at Universidad del Zulia in Maracaibo, then came to the United States and completed her internal medicine residency at Montefiore New Rochelle in New York, caring for a wide range of patients across hospital and outpatient settings.

It was during those residency years that she was drawn to vein care. She repeatedly saw patients who had lived for years with varicose veins, pain, and swelling without ever being told those symptoms could stem from venous insufficiency. She recognized how common the condition was and how significantly it affected patients' quality of life.

What appealed to her was the rare combination the specialty offers. Vein care lets her lean on her internal medicine background, build the lasting patient relationships she values, and perform hands-on procedures that bring relief patients can feel.

What I love about vein disease is that it lets me combine the parts of medicine I enjoy most. I provide comprehensive care and long-term relationships in internal medicine, and I can also perform procedures that help patients feel better. The best part is when patients come back after treatment and tell me how well they feel.

Specialized Training That Sets Her Apart

Not every vein doctor is fellowship-trained, and that distinction matters. Dr. Guillen Aguirre completed her fellowship in venous and lymphatic medicine at Center for Vein Restoration (CVR), in its ABVLM-accredited program directed by renowned vein expert Dr. Peter J. Pappas. Over that year, she trained alongside experienced mentors, learned the full range of minimally invasive vein procedures, and shaped her own approach by drawing on what she learned from each of them.

She is among a small number of physicians fellowship-trained specifically in venous and lymphatic medicine, and CVR's program is one of only a handful of its kind in the country. For patients in Waldorf, that means care from a specialist whose work centers on veins.

During my training, I worked alongside physicians who were not only excellent clinicians but genuinely compassionate. I wanted to build my career in a place that shares those same values.

Care in English, Spanish, and in Sign Language

For many patients, being understood is the first step toward trusting their care. Dr. Guillen Aguirre is fluent in Spanish and treats Spanish-speaking patients in their native language, so nothing about their symptoms, diagnosis, or treatment plan has to pass through an interpreter.

She also connects with patients in a way few physicians can. The daughter of two deaf parents, Dr. Guillen Aguirre grew up communicating in sign language. She is fluent in Venezuelan Sign Language and can communicate with patients in American Sign Language, making her care more accessible to Deaf and hard-of-hearing patients, who so often navigate the healthcare system without a provider who signs. It was an early lesson in meeting people where they are, and it still shapes how she connects with patients today.

A Philosophy Built on Listening

Ask Dr. Guillen Aguirre how she practices, and the answer is consistent: listen first, explain clearly, and treat with compassion. She starts every visit by understanding what the patient is experiencing and what they hope to get back to, then takes the time to make the medicine make sense.

So many patients tell me, 'You are the first person who ever explained why this is happening to me.'

Dr. Caroly Guillen Aguirre

She holds herself to that standard every single day, an expectation she once described by comparing medicine to the stage:

In New York, I loved watching Broadway performers give their absolute best every single night. For us it is the same. We may have a hard day, but our patients are looking for the best in us, and they deserve it every time.

Helping Nervous Patients Feel at Ease

Many people feel anxious before their first vein appointment, often because they are unsure what to expect. Dr. Guillen Aguirre sees calming that worry as part of her job. She meets each patient wherever they are, whether they need someone to listen, someone to answer their questions, or simply reassurance that they are in capable hands.

I will be whatever the patient needs me to be. If they come in scared and unsure of what is happening, I want to be the doctor who reassures them and helps them through it safely, so they know they are in good hands.

Modern vein treatments are minimally invasive and performed right in the office as outpatient procedures. Most involve little to no downtime, and patients can usually return to their normal routine soon after.

How Dr. Guillen Aguirre Measures Success

For Dr. Guillen Aguirre, a good outcome begins long before any procedure. It starts at the very first visit, when a patient finally feels heard and understood.

Success starts on the first day, before we do anything. It is when patients leave the consultation happy, with their questions answered, telling me I am the first person who explained what is happening to them. Then, after treatment, they see and feel the difference: their legs look better, feel better, and the pain that was affecting their life is gone.

She is especially moved by patients who had all but given up. Some have lived with venous ulcers or symptoms so bothersome they stopped going out, self-conscious about how their legs looked and felt. Helping them feel comfortable and confident again is what she finds most rewarding.

Seeing the change you can make in someone's life, watching them feel better, is the most rewarding part of what I do.

Vein Conditions Dr. Guillen Aguirre Treats

Dr. Guillen Aguirre evaluates and treats the full range of venous and lymphatic conditions, including:

Many patients are surprised to learn that end-of-day heaviness, ongoing swelling, and restless, cramping legs at night are not simply the price of aging or long hours on their feet. More often, they are signs of treatable vein disease.

A Trusted Partner for Referring Physicians

Before she specialized in veins, Dr. Guillen Aguirre practiced internal medicine and referred patients to specialists herself. She knows exactly what a referring physician hopes for when they send a patient elsewhere, and she brings that perspective to every referral she receives.

Because I was a referring physician myself, I know how much trust goes into sending a patient to someone else. I take that responsibility seriously. I see this care as a partnership, and it depends on good communication and collaboration.

She keeps communication open from the first visit through the final follow-up, so referring providers stay informed every step of the way.

Beyond the Vein Clinic

Outside of medicine, family comes first for Dr. Guillen Aguirre. After years apart, she recently reunited with her identical twin sister, and she treasures time with her sister, her husband, and her daughter. She is also a passionate home cook whose kitchen spans continents, from the Venezuelan dishes she grew up with to the Persian food she makes for her family. A lifelong vegetarian, she has never eaten meat herself, though she happily cooks it for the people she loves, which tells you plenty about how she shows care. When she is not in the kitchen, she enjoys reading and spending time outdoors, drawn to mountains and natural beauty.

Frequently Asked Questions

Q: What vein conditions does Dr. Guillen Aguirre treat, and do I need a referral?
A: Dr. Guillen Aguirre treats chronic venous insufficiency, varicose and spider veins, leg swelling and edema, venous ulcers, leg heaviness and fatigue, restless legs, and nighttime cramping, along with other venous and lymphatic conditions. You do not need a referral to schedule a consultation. If your condition meets medical necessity criteria, CVR's team can help guide you through the insurance process. Consultations are available in both English and Spanish.

Q: I have had leg symptoms for years and assumed it was just aging. Is it worth getting checked?
A: Yes, and the earlier the better. Vein disease usually develops slowly, so many people adjust to feeling worse over time without realizing the cause can be treated. An evaluation can pinpoint what is happening and lay out clear options, and early care can help keep the condition from progressing.

Q: What should I expect at my first appointment with Dr. Guillen Aguirre?
A: Your first visit is a consultation, not a commitment to treatment. Dr. Guillen Aguirre will listen to your symptoms, review your health history, and perform a thorough evaluation that usually includes a diagnostic ultrasound to assess your blood flow and identify the root cause of your discomfort. She will explain what she finds in plain language and answer your questions, so you leave understanding your condition and your options.
