“Tarsal pain” describes discomfort involving the tarsal region, the group of bones and surrounding tissues between the lower leg and the midfoot. Pain may be felt along the inner ankle, heel, arch, or top of the foot, depending on the affected structure.
The tarsal area contains joints, ligaments, tendons, blood vessels, and the posterior tibial nerve. Many different structures pass through this compact space so that several conditions can produce similar symptoms. Sprains, arthritis, tendon injuries, stress fractures, nerve compression, and swelling can all cause pain in this region.
Vein disease doesn’t usually injure the tarsal bones directly. However, chronic venous insufficiency can allow blood to pool in the lower legs, increasing pressure and causing swelling around the ankles and feet. That swelling may create aching, tightness, tenderness, or added pressure near nerves and soft tissues in the tarsal region.
Pain from a musculoskeletal injury often follows a specific event or worsens with a particular movement. Vein-related discomfort is more likely to occur with changes elsewhere in the lower leg and may become more noticeable after long periods of standing or sitting.

Healthy leg veins use one-way valves to help move blood upward toward the heart. Chronic venous insufficiency (CVI) occurs when those valves weaken or become damaged, causing blood to flow backward and collect in the lower leg.
The ankle is especially vulnerable because gravity increases pressure in the lowest parts of the leg. Fluid may leak into nearby tissues, leading to swelling, also called edema. As tissue pressure rises, the ankle can feel stiff, full, or sore. Shoes may fit more tightly by evening, and normal movement may become uncomfortable.
Swelling may also aggravate an existing foot or ankle condition. For example, a person with arthritis, tendon irritation, or tarsal tunnel syndrome may notice worse symptoms when surrounding tissues become congested. This doesn’t mean venous insufficiency caused the original condition, but it may increase discomfort and complicate recovery.
A careful diagnosis matters because vein disease is only one possible contributor. Tarsal tunnel syndrome occurs when the posterior tibial nerve is compressed near the inner ankle, Johns Hopkins Medicine explains. It often causes burning, tingling, numbness, or shooting pain into the sole of the foot.
Other causes include posterior tibial tendon dysfunction, plantar fasciitis, arthritis, gout, stress fractures, trauma, and poorly fitting footwear. Peripheral artery disease can also cause lower-extremity pain, although it involves reduced arterial blood flow rather than impaired venous return. Diabetes-related nerve damage may create burning or numbness that resembles nerve compression.
A clinician may assess your symptom pattern, examine the foot and ankle, review skin and vein changes, and recommend imaging or nerve testing when needed. If venous disease is suspected, a duplex ultrasound can evaluate blood flow and valve function in the leg veins.
Treatment should address the confirmed cause rather than pain alone. For mild venous symptoms, a clinician may recommend regular walking, calf-muscle exercises, leg elevation, weight management, and avoiding long periods of immobility. These measures support circulation and may reduce pressure in the lower legs.
Compression stockings may help control swelling, but they aren’t appropriate for everyone. People with certain arterial circulation problems need medical guidance before using compression. Proper sizing also matters, since poorly fitted garments may be uncomfortable or ineffective.
When diseased superficial veins are driving symptoms, minimally invasive outpatient vein treatments may be recommended. According to the American Venous Forum, options can include endovenous ablation, sclerotherapy, or other procedures that close malfunctioning veins, depending on the ultrasound findings. Blood then reroutes through healthier veins. Orthopedic, podiatric, or neurologic care may still be necessary when another condition is present.
Schedule an evaluation when tarsal pain lasts more than a few days, keeps returning, limits walking, or occurs with persistent swelling or skin changes. Prompt assessment is also important for numbness, weakness, an open sore, or pain that is steadily worsening.
Sudden swelling in one leg, especially with warmth, redness, or tenderness, can signal a deep vein thrombosis. Seek urgent medical attention. Chest pain, shortness of breath, fainting, or coughing blood may indicate a pulmonary embolism and requires emergency care.
Tarsal pain can arise from several systems, and symptoms sometimes overlap. The most useful next step is an evaluation that considers orthopedic, nerve, and circulation-related causes instead of assuming the problem comes from one structure.

Can vein disease affect only one ankle?
Yes. Venous insufficiency may be more advanced in one leg because of previous blood clots, injury, anatomy, or differences in valve damage. Uneven swelling still deserves medical evaluation.
Does footwear cause venous insufficiency?
Footwear doesn’t cause damaged vein valves. However, tight shoes may feel more uncomfortable when ankle or foot swelling is present and can make pressure-related symptoms easier to notice.
Can dehydration cause tarsal pain?
Dehydration may contribute to muscle cramping, but it doesn’t typically explain persistent focal ankle pain, swelling, skin changes, or visible varicose veins. Ongoing symptoms should be assessed rather than attributed to hydration alone.
